Continuity of Care is Important!

PLEASE AVOID OTHER WALK-IN CLINICS!

Continuity  of care means having comprehensive medical care provided to you in a  coordinated manner by your family physician. Over a long period of time,  we develop a physician – patient relationship with you, and this  combined with in-depth knowledge of your health allows us to make  medical decisions that that are in your best interest.

When our patients go to walk in clinics, continuity of care becomes disrupted:

  • Most  walk-in clinic doctors do not have your complete medical history. They  do not have an established relationship with you to guide medical  decision-making.
  • Your  family physician will not know what happened at the walk–in clinic,  what tests were ordered, or what treatment/referrals were made
  • Any results or reports generated from the visit will return to the walk-in clinic and not to your family physician
  • Walk-in clinics will not arrange for follow up of ongoing issues

We work together as a team to provide after hours  coverage so our patients do not need to go to walk-in clinics. We  provide an after hours Clinic Monday to Friday from 5 – 8pm, and on  Saturdays from 9am– 12 noon, for urgent issues only.

When you attend the After Hours Clinic:

  • You will be seen by your family physician or one of the doctors in our team
  • Be  assured that your family physician will have the full story from your  visit to after hours clinic visit including any results/reports generated from  the visit

Purpose of the After Hours Clinic

If you become ill after hours and need care, one of the doctors in our FHO  will be happy to see you. There are many benefits to using the FHO  clinics instead of a walk-in clinic:

  • Your own doctor will be made aware of your visit and can therefore follow up on tests ordered or referrals if necessary
  • It ensures continuity of care

What the After Hours Clinic is not used for

The After Hour Clinics Are Not Meant for Life Threatening Emergencies

If you have chest pain, if you think you are having a stroke, heavy bleeding, or fracture, severe allergic reaction or trouble breathing, call 911 immediately or go directly to the nearest emergency department.

Generally You Should Not Use The After Hours Clinics for the Following:

  • Routine blood pressure checks. We will see urgent cases
  • Regular prescription renewals. Ideally this should be done by your doctor during working hours.
  • Results of routine lab tests or procedures ordered during working hours.
  • Notes for work or absences due to illness (if an exception is made,  charges may apply)
  • Requests for referrals, procedures or for appointments with your own doctor
  • Pre-Operative physicals, routine paps or Well baby visits as these are more appropriately done by your regular Family Doctor

Frequently Asked Questions

If you have new worrisome symptoms, a child that is ill, or worsening of a  chronic medical condition, which you feel cannot wait until a routine  appointment with your family physician, you will be assessed. However,  your visit is not meant to replace the complex and continuing care that  only your family doctor can provide. Remember, the after hours clinic is  meant to provide for urgent non life-threatening problems only.

Appointments cannot be booked in advance. Services are provided on a “first come, first serve” basis.

Due  to high demand for this service, the clinic doctors are only  able to address one immediate problem per patient, per visit.

No. The After Hours Clinic runs after hours only.  We will start seeing patients at 5pm on weekdays and 9am on Saturdays. The clinic is closed on Sundays and Public holidays.

No, This clinic is strictly for after hours  urgent care. Clinic doctors do  not provide on-going “full-service” medical care. Pap smears, physicals,  breast examinations, vaccinations, psychotherapy, or counseling are  better delivered by your regular family health care provider.

We  strive to provide care to all the patients needing urgent medical care.  The demand for care sometimes exceeds the capacity of the After Hours  Clinic. Patients are asked to please arrange an appointment with their  family doctor whenever possible. Patients who have needs that are non-urgent, such as medication refills, are also asked to please contact  their own family physician directly.

Patients with emergency health concerns should call 911 or report to the nearest hospital’s emergency department.

Please contact the Magnolia Health Center during business hours to register with a family physician.
